Structured ED Triage Assessment
Triage Data Entry
- Date of birth / Age
- Sex
- Examination date & time
- Triage system
- Chief complaint
- Heart rate
- Respiratory rate
- SpO₂
- Oxygen
- Hypercapnic respiratory failure
- Systolic BP
- Temperature
Document the triage priority with:
- Chief complaint
- Key vital signs (HR, BP, SpO₂, RR, T)
- Risk factors (age, comorbidities, medications)
- Immediate or anticipated interventions
- Explicitly link these to the chosen triage category (e.g., “Orange due to tachypnea, hypoxia and chest pain”).
Summarize a Bayes-type approach:
- Pre-test probability (history, prevalence, presentation)
- How findings/tests shift probability
- Post-test probability for key diagnoses (e.g., ACS, PE, Sepsis)
- Use indicative ranges (low <10%, intermediate 10–30%, high >30%) when appropriate
Ask the AI to apply and interpret appropriate tools, such as:
- qSOFA: suspected sepsis (RR, GCS, SBP)
- HEART: chest pain / suspected ACS
- Wells / PERC: suspected PE
- CHA₂DS₂-VASc / HAS-BLED: atrial fibrillation
Prompting Techniques

Copy-ready prompt
Click Generate Prompt to create the complete text. Required fields are listed above.
You are an ED triage nurse with expertise in triage systems and protocols, operating in a high-acuity, high-volume environment. Your assessments prioritize clinical accuracy and real-time prioritization.
Task
Assign an appropriate triage priority based on the selected triage system (e.g., ESI, CTAS, MTS), comment on risk factors, and recommend next steps.
Safety & governance
De-identification activeAI-ready prompt exports exclude dedicated identifiers and automatically redact explicit identifiers and common identifier patterns found in free text. Review the exported prompt before sending it outside your clinical environment.
